2. EPA can require treatment, storage and disposal (TSDF) facilities handling hazardous waste to undertake corrective actions to clean up spills resulting from failure to follow hazardous waste management procedures or other mistakes. The process includes cleanup procedures designed to guide TSDFs toward in spills.
3. The use of Director-approved methods to assure that wells within the area of review do not serve as conduits for the movement of fluids into USDWs.
4. Any measures taken to rectify conditions adverse to quality and, where possible, to prevent recurrence.
5. Cleanup of hazardous waste contamination at non-Superfund sites.
6. An action taken to eliminate the causes of an existing nonconformity, defect, or other undesirable situation in order to prevent recurrence.
7. The many actions that EPA undertakes in addressing the investigation, and remediation of contamination at or from hazardous waste treatment, storage or disposal facilities.
8. An EPA program to address the investigation and cleanup of contamination from solid and hazardous waste facilities.
9. Implementing procedures that are based on lessons learned from actual incidents or from training and exercises.
10. Design, operation and maintenance changes that one takes consistent with good engineering practice to reduce or eliminate the likelihood of the recurrence of the primary cause and any other contributing cause(s) of an event identified by a root cause analysis as having resulted in a discharge of gases to an affected flare in excess of specified thresholds.
11. Design, operation and maintenance changes that one takes consistent with good engineering practice to reduce or eliminate the likelihood of the recurrence of the primary cause and any other contributing cause(s) of an event identified by a root cause analysis as having resulted in a discharge of gases from an affected facility in excess of specified thresholds.
