Phase II Assessment

Phase II Report Completion date(s) are the date(s) on which any Phase II reports (e.g., supplement assessment, site investigation) were completed. A Phase II report is final when an environmental professional or state official has prepared an environmental assessment report that has been accepted by the grant recipient. If multiple Phase II reports are completed for the property during the term of the grant they should all be reported to EPA.
