Case management

A collaborative process of assessment, planning, facilitation, and advocacy to meet an individual's health needs through communication and available resources. Case managers, who are often nurses or social workers, work with patients and their families to coordinate care for complex, catastrophic, or chronic conditions. The process is guided by standards from bodies like the Case Management Society of America (CMSA).
