A systematic process for identifying the fundamental reasons for an undesirable event or problem, rather than just addressing its symptoms. In risk management, it involves tracing back through a chain of causation to pinpoint the underlying factors that, if corrected, would prevent recurrence. This analytical approach is applied to incidents such as accidents, operational failures, or quality control issues to develop effective corrective actions.
