An actual or alleged error, misstatement, misleading statement, act, omission, neglect, or breach of duty committed by an insured person or entity. It forms the basis for claims under professional liability, directors and officers (D&O), or errors and omissions (E&O) insurance policies. Such acts can lead to legal liability and financial damages for which coverage may be sought.
