Balance billing

The practice by a healthcare provider of charging a patient for the difference between the provider's usual fee and the amount reimbursed by the patient's insurance plan. This occurs when the provider is not contracted with the insurer and the insurer's payment is less than the billed amount. Many states have laws restricting or prohibiting balance billing, especially for emergency services.
