Evidence of insurability

Documentation or information an applicant provides to demonstrate an acceptable level of risk for life or health insurance. This includes medical history, results from physical exams, and details about lifestyle choices such as smoking or participation in hazardous activities. Insurers require this evidence for applications involving higher coverage amounts, for older applicants, or for those applying outside of a group's open enrollment period.
