Health care proxy

A legal document that designates another person, known as an agent or surrogate, to make healthcare decisions on an individual's behalf if they become incapacitated. This proxy's authority typically begins when the individual's attending physician determines they lack the capacity to make their own medical choices. The designated agent must act in accordance with the individual's known wishes or best interests.
