Health care directive

A legal document that allows an individual to make decisions about their medical treatment in advance, should they become unable to communicate their wishes. This directive typically includes a living will, specifying desired medical interventions, and a durable power of attorney for healthcare, designating an agent to make decisions. It becomes effective upon a physician's determination of the individual's incapacity.
