Out-of-pocket costs

The portion of a covered loss or expense that a policyholder must pay personally, which is not reimbursed by the insurance plan. These costs include deductibles, coinsurance, and copayments for covered services, as well as any charges for services that are not covered by the policy. Health insurance policies specify an out-of-pocket maximum, which is the most a policyholder has to pay for covered services in a plan year.
