Epididymal appendix

A small, stalked cystic structure attached to the head of the epididymis, representing a vestigial remnant of the embryonic mesonephric (Wolffian) duct. This benign anatomical variation is present in a fraction of the male population and is homologous to the paratubal cysts found in females. It is typically asymptomatic but can occasionally undergo torsion, leading to acute scrotal pain that mimics testicular torsion.
