Intrinsic clearance

The inherent capacity of an organ, usually the liver, to remove or metabolize a drug when blood flow and protein binding place no limit on delivery, reflecting the efficiency of its enzymes and transporters, roughly Vmax divided by Km at low drug concentration. It sets hepatic clearance for low-extraction drugs, whereas the clearance of high-extraction drugs approaches liver blood flow and changes little with intrinsic clearance.
